Cardioprotective effect of calcitonin preconditioning against ischemia reperfusion injury

Abstract : This study was carried out on 40 albino rats of both sexes to investigate the effect of calcitonin hormome on cardiac vulnerability to ischemia and reperfusion in isolated hearts. Rats were allocated into 4 groups. Single dose calcitonin treated group : ten rats injected intraperitoneally with calcitonin hormone in a dose of 25 IU/ml/kg body weight. Group 2 : ten rats inject'ed with distilled water and used as control for group I. Group 3: 10 rats subjected to daily intraperitoneal injection of calcitonin for 2 weeks. Group 4: ten rats injected with distilled water and used as control for group 3. Animals were then sacrified and the hearts were removed. The isolated hearts were perfused using Langendorff technique and examined for basal activities. Then, hearts were subjected to 30 minutes ischemia followed by 30 minute period of reperfusion. In single dose calcitonin-treated rats, the baseline preischemic activities of the isolated hearts were not significantly changed as compared to control group. The postischemic reperfusion values of peak developed tension (PT) and coronary flow rate (CFR) were significantly higher in single dose calcitonin-treated animals at 30 minute of reperfusion (P< 0.01). On the other hand the time to peak tension (TPT), half relaxation time (1I2RT) and heart rate (HR) were not significantly changed. This positive inotropic effect of calcitonin during reperfusion period was demonstrated by a lower % change from preischemic values (-32% at the end of reperfusion compared to -49% in control rats P<0.02). In 2 weeks calcitonin-treated rats, the baseline activities showed significantly higher PT and CFR compared to the control group (P<0.02). Throughout the 30 minutes of reperfusion the PT was significantly higher in calcitonin-treated rats (at least P<0.05). Similar pattern was observed in CFR. Whereas TPT and 1/2 RT were significantly shorter in calcitonin- treated rats compared to control ones during different periods of reperfusion. Calitonin injection was associated with positive chronotropic effect represented in a significantly higher RR during reperfusion period at 15- 25¬& 30- minutes (at least P<0.05). The protective effect of calcitonin during reperfusion period was manifested by a lower percentage change in all parameters studied which reached significant levels throughout the 30 minute reperfusion. In addition, 2 weeks calcitonin-treated rats showed also significant lowering of creatine kinase (CK) and lactate dehydrogenase LDH) in coronary effluent during postischemic reperfusion period compared to the l;ontrol group. In this study, calcitonin per se and/or the induced hypocalcemia protects against ischemia reperfusion damage by creating a preconditioning like state in the isolated rat heart.
