Quality of Life among Osteoarthritis Patients in Makkah: A cross-sectional study

Abstract : Background: Osteoarthritis (OA) is the most common chronic joint disease. In the absence of an effective medical treatment and due to the chronic nature of this condition, medical diagnosis may result in affecting the health-related quality of life. Therefore, the aims of this study are; assess quality of life among osteoarthritis patients using generic questionnaire SF-36, Investigate factors affecting quality of life among osteoarthritis patients, and evaluate the association between OA disease severity and QoL among OA patients attending three general hospitals in Makkah. Methods: Subjects (n =320) were approached as participants of hospital-based cross-sectional study. Diagnosis of osteoarthritis was obtained from orthopedics and rheumatologist's consultant. Median and IQR were used to report the finding. Linear regression was used to find the predictors of QoL domains. The eight dimensions of health-related quality of life; physical functioning, role limitation due to physical health, emotional well-being, role limitation due emotional problem , social functioning, pain, energy, and general health domains were evaluated using the Medical Outcomes Study: 36-Item Short Form Survey. Results: Three hundred and twenty OA patients participated in this study. Their age ranged from 23 to 98 years (57.09±12.45 years) majority were females (81.88%). Role limitations due to physical health domain showed the lowest median (25), and it was the most affected domain by the disease, followed by the energy and fatigue (45) ,pain domain (47.5), general health(50), physical functioning domain (50), Social functioning (62.5) Emotional well-being (64) , and Role Limitations due to emotional problems (66.66). Age had a negative impact on Qol of OA patients. Disease severity and disease duration were statistically significant association with all SF-36 domains (p<0.001).Linear regression showed Age, education level, income, disease severity, chronic diseases, and physiotherapy were the predictors of QoL. Conclusions: OA had a negative impact on patient's quality of life. The physical health was affected more than the emotional and social health. This finding attributed to find other therapeutic way to enhance the OA quality of life. Physiotherapy might be obtained to all OA patients to maintain their pain and joint stiffness. Further researches should be conducted to improve the Qol of this type of chronic disease and to predict other risk factors for Saudi patients.
