Abstract : This study conducted a retrospective study at Khamis Mushit Hospital Radiological Department to determine reasons for rejecting/repeating procedures with a focus on the protection of patients from preventable irradiation. The main objective of the study aimed at establishing quality control issues on digital radiation at the facility. Positioning error, machine problems, anatomy cutoff, artifacts, and body movements contributed to frequencies of repeat irradiation in the respective order. Overall, staff at the facility had a significant contribution to the reject/repeat rates. Senior and junior staff accounted for 87.5% of repeat procedures at the facility, while staff with expert proficiency contributed to 12.4% of the reject/repeat procedures. The study proposes the establishment of quality control measures like practical staff guidance and on-the-job training and quality control measures to heighten staff awareness and improve operational skills at the facility to improve patient safety from preventable irradiation.
